Konfigurace hrudníku u pacientů s depresivními poruchami a její ovlivnění pomocí fyzioterapie

Abstract

Depressive disorder occurs worldwide predominantly from 6 to 20 %. However, the exact percentage value cannot be precisely given. In a major of patients, unfortunately, depressive disorder remains undiscovered and cannot be diagnosed as well as treated properly. Annually, the number of patients with depressive disorder increases linearly. The depressive disorders can be split in primary and secondary occurrences based on different internal diseases. Moreover, there are also a great amount of somatic manifestations associated with depression. This bachelor thesis focuses on these manifestations, specifically on the chest area and its connection with problematic posture, breathing stereotype and depression. The theoretical part of this thesis describes the posture and breathing functions influenced by the patient's mental state. Additionally, it also deals with physiotherapeutic methods that are able to break the vicious circle of deteriorating postural and breathing functions accompanying changes in the patient's mental health. Practical part includes a case report of three patients. One suffers from depression due to late diagnosis of cystic fibrosis and afterbirth depression. Depresivní porucha se světově vyskytuje s prevalencí od 6 do přibližně 20 %. Přesné procento není možné udat. U většiny pacientů zůstává depresivní porucha nerozpoznána a není ji možné diagnostikovat a správně léčit. Každý rok pacientů s depresivní poruchou lineárně přibývá. Výskyt deprese se dělí na primární a sekundární, která je důsledkem jiného interního onemocnění. S depresí se pojí i množství somatických projevů. Bakalářská práce se zaměřuje na tyto somatické projevy, konkrétně na oblast hrudníku a její spojitost s problematickou posturou, dechovým stereotypem a depresí. Teoretická část práce popisuje posturu a dechové funkce, které jsou ovlivněny psychickým stavem pacienta. Dále se zabývá fyzioterapeutickými metodami a přístupy, které mohou přerušit bludný kruh zhoršujících se posturálních a dechových funkcí se změnami psychického stavu pacienta a naopak. Praktická část obsahuje kazuistiku tří pacientek. První z nich trpí depresivními stavy v důsledku pozdní diagnostiky cystické fibrózy a poporodních depresí. Druhá pacientka se potýká s depresivními stavy v důsledku náročné životní situace. Třetí pacientka trpí kombinací depresivní a schizoafektivní poruchy bez známých příčin.
