Rodina a její role ve vnímání současné zdravotnické etiky

Abstract

Diplomovápráce se zabývá tématem rodiny a přístupu k ní v kontextu současné zdravotnické etiky. V úvodupopisuje vnímánírodinyz pohleduvybranýchhumanitnícha společenských věd a stanovuje prvky, které jsou společné pro všechny obory včetně zdravotnictví. První část se pak zabývá vývojem vztahu mezi lékařem, pacientem a jeho rodinou a to od paternalistickéhovztahupřesinformovaný souhlas až po dříve vyslovené přání. Jednotlivé instituty dává do spojitosti s legislativními i etickými normami a poukazuje na dilemata v jejich chápání. Současně řeší možnosti zapojení rodiny v každém z nich. Druhá část práce se věnuje tématu paliativní péče. Popisuje základní principy u dospělých pacientů v terminálním stádiu nemoci a dále postupy v dětské i perinatální paliativní péči. V závěru pak představuje prakticképostupypři zaváděnípéče ve Fakultnínemocnici Královské Vinohrady v Praze. Součástí jsou i kazuistiky z působení paliativního týmu.

The diplomathesisdiscussesthe issue of familyandthe approach to family in the context of current medical ethics. The introductiondescribesthe perceptionof familyby humanities and social sciences and identifies the elements shared by all disciplines, including health care. The firstsectionof the thesislooksintothe development of the relationship between the physician and the patientandhisor her family,rangingfromapaternalisticrelationshiptoinformedconsentto patients' previously expressed wishes. The author places each of these principles in context with legislative andethical normsandhighlightsthe dilemmasassociatedwiththe understandingthereof. At the same time it solves the possibilities of family involvement in each of them. The conclusion offers a description of the practical procedures followed whenthiscare wasbeingintroducedatthe Královské VinohradyUniversityHospital inPrague.Partof the thesis is also casuistry derived from the operation of the palliative care team.
