uk.abstract.cs | Předložená práce se zabývá v současnosti velmi aktuální problematikou připravenosti
nemocnic na řešení krizových situací při hromadných neštěstích se zaměřením na teroristické
útoky. V práci je popsán charakter teroristických útoků, specifika zranění bombových útoků a
střelných poranění a nejčastější komplikace spojené s příjmem velkého počtu pacientů do
nemocničního zařízení.
V úvodu práce jsou vysvětleny nejdůležitější odborné termíny, především triage, její
druhy a kategorie. Stručně jsou popsány principy jednotlivých metod a postupů třídícího
systému, jsou uvedeny rizikové faktory a omezení použitelnosti třídících metod s ohledem na
charakter neštěstí a možnosti zdravotnického zařízení. Ověření účinnosti jednotlivých postupů
je doloženo aplikací v řadě reálných situací.
V následujících kapitolách jsou detailně popsány autorovy osobní zkušenosti z mnoha
zahraničních misí, rozdílné postupy při triagi v jednotlivých situacích a rozdílné aplikace
jejích kategorií. Působení v misích je rozděleno podle typu hromadného neštěstí do oblastí
postižených válečným konfliktem nebo přírodní katastrofou a chudobou. Zkušenosti z misí
jsou doplněny odbornými poznatky ze studijní stáže v Izraeli – v zemi s vysokou četností
teroristických útoků. Nadstandardně je hodnocena připravenost a vybavenost izraelských
nemocnic, které tak slouží tak jako modelový příklad pro organizaci našich zdravotnických
zařízení.
Aktivace traumatologického plánu a připravenost nemocnic na řešení krizových situací
byly konzultovány rovněž se specialisty ve významných evropských traumacentrech. Na
základě výše uvedených dlouhodobých zkušeností byla navržena a na 1. chirurgické klinice
VFN v Praze provedena simulace aktivace traumatologického plánu za přítomnosti auditorů.
V práci je průběh cvičení detailně popsán, jsou presentovány poznatky z aktivace a
analyzovány výsledky auditorů včetně krizových situací simulace.
Klíčovou částí práce je dotazníkové šetření 27 zahraničních i domácích respondentů a
velmi dobře ilustruje stav této problematiky v různých zemích světa. Soubor je rozdělen na
zdravotnická zařízení ve vyspělých a rozvojových státech světa podle indexu lidského rozvoje
OSN, další podskupinou pak tvoří nemocnice v oblastech s vyšší četností teroristických útoků
a samostatnou skupinou je 6 nemocnic v České republice. Na základě výsledků
dotazníkového šetření je porovnávána připravenost nemocnic na řešení hromadných neštěstí

v obou skupinách podle vyspělosti země a dále podskupina nemocnic v České republice a
v zemích s častými teroristickými útoky. Data získaná dotazníkovým šetřením jsou
vyhodnocena statistickými metodami.
Následující diskuse a vyhodnocení, spolu s osobními zkušenostmi a důkladnou
znalostí reálných řešení se staly podkladem pro doporučení postupů triage, organizace
chirurgických a traumatologických oddělení a materiálního vybavení zdravotnických zařízení
v České republice.
Aplikací výsledků habilitační práce lze dosáhnout optimálního řešení a zvýšení
spolehlivosti v připravenosti nemocnic při řešení krizových situací, zejména odstranění
diskrepance mezi teorií a realitou. | cs_CZ |
uk.abstract.en | This thesis presents the very current topic of general hospital preparedness to deal with
crisis situations in mass casualty incidents with a focus on terrorist attacks. The thesis
describes the nature of terrorist attacks, specifics of bombing and gunshot wounds and the
most common complications associated with admitting a large number of patients to a
hospital.
The introduction explains the most important technical terms, especially triage, its
types and categories. The principles of individual methods and procedures of the triage
system are briefly described, risk factors and limitations of the applicability of triage methods
with respect to the nature of the disaster and the possibilities of the medical facility are given.
The verification of the effectiveness of individual procedures is documented by the
application in a number of real situations.
The following chapters describe in detail the author's personal experience from many
humanitarian missions, the different procedures for triage in individual situations and the
different applications of its categories. Missions are divided according to the types of mass
casualty incidents into areas affected by war or natural disaster and poverty. The experience
of the missions is complemented by expertise from a study stay in Israel - a country with a
high frequency of terrorist attacks. The preparedness and equipment of Israeli hospitals is
assessed above standard and thus serves as a model example for the organization of our
medical facilities.
Specialists in major European trauma centres were also consulted on the activation of
the disaster response plan and the preparedness of hospitals to deal with crisis situations.
Based on the above-mentioned long-term experience, a simulation of the disaster alert
notification in the presence of auditors was designed and performed at the 1st Surgical Clinic
of the General University Hospital in Prague. The work describes the course of the exercise in
detail, presents the knowledge of activation and analyses the results of auditors, including
crisis points during simulation.
The key part of the work is a questionnaire survey of 27 foreign and domestic
respondents, which illustrates the state of this issue in various countries of the world very
well. The group is divided into health facilities in developed and developing countries of the

world according to the UN Human Development Index. Another sub-group consists of
hospitals in areas with a higher frequency of terrorist attacks and a separate group is formed
by 6 hospitals in the Czech Republic. Based on the results of the questionnaire survey, the
preparedness of hospitals for dealing with mass casualty incidents in both groups is compared
according to the maturity of the country and a subgroup of hospitals in the Czech Republic
and in countries with frequent terrorist attacks. Data obtained by the questionnaire survey is
evaluated using statistical methods.
The following discussions and evaluations, together with personal experience and
thorough knowledge of real solutions, became the basis for recommendations of triage
procedures, organization of surgical and traumatology departments and material equipment of
medical facilities in the Czech Republic.
By applying the results of the habilitation thesis, it is possible to achieve an optimal
solution and increase the reliability in the preparedness of hospitals in dealing with crisis
situations, especially the elimination of discrepancy between theory and reality. | cs_CZ |
