Abstract

Těhotenství u žen s diabetem mellitus 1. typu s sebou přináší řadu komplikací a rizik pro matku i dítě během těhotenství, porodu i po porodním období. Proto je důležité, aby pacientky byly správně léčeny a onemocnění bylo dobře kompenzováno. Výborná kompenzace diabetu a přidružených onemocnění před a v průběhu těhotenství je základem úspěšného těhotenství. Těhotenství by v ideálním případě mělo být plánované v době co nejlepší kompenzace diabetu. Léčba diabetu v těhotenství zahrnuje úpravu životního stylu (diabetická dieta a pohyb), pravidelný selfmonitoring glykémie a inzulinoterapii pomocí inzulínových per či inzulínovou pumpou. Diplomová práce se zabývá problematikou diabetu mellitus 1. typu v těhotenství. Cílem práce bylo porovnat vliv léčby inzulínovými pery vs inzulínovou pumpou na kompenzaci diabetu, průběh těhotenství, porodu a komplikace u plodu. V teoretické části práce jsou shrnuty aktuální informace a poznatky o diabetu mellitus 1. typu v těhotenství. V praktické části je porovnávána léčba inzulínovou pumpou a inzulínovými pery. Jednalo se o statistickou analýzu dat u žen léčených inzulínovými pery či inzulínovou pumpou. Pregnancy in women suffering from type 1 diabetes mellitus is associated with various complications and risks for both the mother and the baby both during pregnancy as well as during the delivery and in the postpartum period. For this reason, it is essential to ensure proper treatment and compensation of the disease. Proper diabetes compensation and treatment of any associated diseases before and during pregnancy is the basis for a successful pregnancy. If possible, pregnancy should be planned to coincide with a period of the best possible diabetes compensation. Treatment of diabetes in pregnancy includes lifestyle modification (diabetic diet and exercise), regular self-monitoring of glycaemia and insulin therapy with insulin pens or an insulin pump. The subject of the thesis is type 1 diabetes mellitus in pregnancy. The aim was to compare the outcomes of treatment with insulin pens vs insulin pump with regard to diabetes compensation, course of pregnancy and delivery and, lastly, foetal complications.
