Maisonneuveova zlomenina - anatomie, diagnostika, operační přístupy a techniky osteosyntézy

Abstract

Maisonneuveova zlomenina (MZ) je v traumatologii hlezna obecně známý pojem, přesto o ní existuje řada nesprávných představ, které mohou vést ke špatné volbě léčebného postupu. Navíc pojem Maisonneuveova zlomenina není všem autory chápán stejně a recentní CT studie potvrdily, že se jedná o mnohem komplexnější a variabilnější poranění. Disertační práce byla rozdělena do dvou hlavních částí, a to anatomické (experimentální) a klinické. V experimentální části bylo cílem popsat výskyt a tvar a prokázat klinický význam crista malleoli lateralis (CML), která se nachází na laterální straně distální fibuly a kterou předchozí anatomické i klinické práce opomíjely. Znalost této struktury je klíčová pro správné umístění syndesmálních šroubů. Cílem klinické části byl přesný popis pathoanatomie jednotlivých lézí v rámci "klasické" i méně častých forem Maisonneuveovy zlomeniny na základě CT vyšetření a peroperačních nálezů. Anatomické studie byly provedeny na 352 suchých kostěných preparátech dospělých fibul. Klinické studie byly provedeny na souboru 54 dospělých pacientů s "klasickou" MZ vzniklém v období 2012-2018 a 11 dospělých pacientů s tzv. "dvojitou" MZ vzniklém v období 2012-2020. Although Maisonneuve fracture (MF) is a well-known type of ankle fracture-dislocation, there are a number of misconceptions that can lead to wrong choice of treatment. The concept of Maisonneuve fracture is also not understood by all authors in the same way and the recent CT studies confirmed that this is much more complex and variable injury. The dissertation was divided into two main parts: anatomical (experimental) and clinical. In the experimental part, the aim was to describe the appearance and shape and demonstrate the clinical significance of the crista malleoli lateralis (CML), which is located on the lateral side of the distal fibula and which previous anatomical and clinical studies neglected. Knowledge of this structure is essential to correct placement of syndesmal screws. The aim of the clinical part was the precise description of the pathoanatomy and associated injuries within the "classic" and less frequent forms of Maisonneuve fracture based on CT examination and intraoperative findings. Anatomical studies were performed on 352 dry bone specimens of adult fibula.
