Kontraktury a jejich profylaxe

Abstract

Cílem této práce je popis problematiky kontraktur z perspektivy studijního oboru ergoterapie. V práci jsou uvedeny příčiny vzniku kontraktur s důrazem na kontraktury horních končetin a dále způsoby, jakými je možné jejich vzniku předcházet. Základem pro správné vyhodnocení vlivu spasticity na vývoj kontraktur je podrobný popis spasticity a výčet možností její léčby. Z inovativních léčebných metod je rozebrána terapie spasticity pomocí botulotoxinu. Podrobně je popsán botulotoxin, jeho složení, indikace a kontraindikace léčby, vlastní léčebná aplikace a druhy léčebných přípravků, jež jsou na trhu k dispozici. Praktická část s obrazovou přílohou popisuje možnosti ergoterapie na třech kasuistikách klientů z Kliniky rehabilitačního lékařství v Praze, kteří jsou po poškození mozku léčeni botulotoxinem. I přes nedostatečnou velikost studovaného souboru se zdá, že jedním z nejdůležitějších faktorů úspěchu této léčby je motivace nemocného k terapii, jeho informovanost a aktivní zapojení rodiny. Léčba botulotoxinem přináší zlepšení funkčního pohybu, avšak bez iniciativy a potřebné spolupráce klienta s terapeutem je tato poměrně nákladná léčba degradována pouze na kosmetické zlepšení vzhledu postižené končetiny.

The main aim of this bacheloťs thesis is to describe the problematics of contractures forming with emphasis on the most common contractures on upper limbs. The processes to prevent the contractures are also described. The secondary objective of this work is to raise knowledge of my colleagues working and studiing in the fíeld of occupational therapy. In my work I wanted to describe the problematics of forming contractures, with placing emphasis on most common contractures on upper limbs and on processes preventing the contractures as well. For the correct evaluation of spasticity influence on contractures creation it is necessary to have its description of and also possibilities of its treatment. Main focus is given to botulotoxin therapy, its composition, indication and counter-indication of treatment, pharmaceuticals currently available on the market and its applications. Practical part with picture section describes possibilities of occupational therapy on three subjects on Department of Rehabilitation Medicine in Prague who had suffered brain damage and are currently on botulotoxin treatment. Just firom this small sample the results seem to indicate, that one of the most important factors of treatment is patienťs motivation; knowledge of treatment and family interest.
