Celkové vyhodnocení dvouletého (2009 - 2010) pilotního projektu vyšetřování funkce štítné žlázy u těhotných žen

Abstract

V letech 2009 a 2010 proběhl v 13 městech České republiky pilotní projekt vyšetřování funkce štítné žlázy. V rámci screeningu vrozených vývojových vad byly ženám v prvním trimestru těhotenství po podpisu informovaného souhlasu vyšetřeny hladiny TSH, FT4 a anti- TPO v séru. Ke stanovení analytů byly použity různé analyzátory, kterými laboratoře disponují a jsou schopné provádět potřebná vyšetření. Většina analyzátorů používá referenční intervaly pro TSH, FT4 a anti-TPO od výrobce. Tyto referenční intervaly jsou vhodné pro běžnou populaci, pro těhotné ženy však již méně. Data z tohoto projektu byla znovu vyhodnocena pomocí nově vypočtených referenčních intervalů, vypočtených nemocnicemi v Praze a Ostravě, pro těhotné ženy, které jsou pro danou skupinu vyšetřovaných žen mnohem vhodnější. Z celkového souboru 3484 těhotných žen bylo díky těmto novým referenčním intervalům zachyceno o 100 těhotných žen se zvýšenou hladinou TSH více, než při použití referenčních intervalů výrobců. Při porovnání referenčních intervalů od výrobce s nově vypočtenými referenčními intervaly pro těhotné se podařilo u hladiny FT4 zjistit o 126 více žen pod hladinou nového referenčního intervalu, než u referenčních intervalů výrobců. In the years 2009 and 2010 the pilot project investigating the function of the thyroid gland took place in 13 cities of the Czech Republic. In the context of the screening of congenital developmental defects the levels of TSH, FT4 and anti-TPO in serum were by women in the first trimester of pregnancy investigated, after signing informed consent. For the determination of analytes there were different analyzers used, which laboratories possess and are able to carry out the necessary examinations. Most of analyzers the reference intervals for TSH, FT4 and anti-TPO has set up from the manufacturer. These reference intervals are suitable for the normal population, neither less for pregnant women. Data from this project was evaluated using the newly calculated reference intervals for pregnant women, which are for the group of investigated women much more appropriate. From the total file of 3,484 pregnant women, it was thanks to these new reference intervals captured about 100 pregnant women with increased levels of TSH more, than in the use of reference intervals of producers.
