Vliv bariatrických operací na diabetes mellitus

Abstract

Bakalářská práce je zaměřena na problematiku obezity, diabetu mellitu a jejím vlivu po provedených bariatrických operací, je rozdělena do dvou částí, teoretickou a praktickou. Teoretická část popisuje problematiku obezity a její léčbu. U komplikací s obezitou je zvláště zaměřena na přidruženou komplikaci, kterou je metabolické onemocnění a to je diabetes mellitus 2. typu. Dále seznamuje s možnými typy bariatrických operací, které mají příznivý vliv na zdravotní stav jedinců s diabetem 2.typu. Nové trendy v bariatrické chirurgii. A také na předoperační a pooperační přípravu k bariatrickému výkonu. V praktické části popisuje vliv bariatrických operací na hmotnost a metabolické parametry pacientů. Byl zkoumán soubor 162 pacientů. Z nich bylo 68 diabetiků a 94 nedabetiků. U obou skupin došlo k výraznému poklesu hmotnosti u všech typů bariatrických výkonů. Na kompenzaci DM 2.typu měl největší vliv operační výkon gastrický bypass. Výsledek potvrdil pozitivní vliv bariatrických výkonů v léčbě obezity. Ať už jde o úbytky hmotnosti a úpravu metabolických parametrů. Zvláště parametrů glykémie a glykovaného hemoglobinu. A tím bylo výzkumem potvrzeno, že bariatrická chirurgie pomáhá v léčbě DM 2. Typu. This bachelor project is focused on the issues of obesity, diabetes mellitus and its influence after the performed bariatric surgery, it is divided into two parts, theoretical and practical. Theoretical part deals with obesity and its treatment, consider the obesity complications, especially the accompanying metabolical complication so called diabetes mellitus of the second type. Moreover it introduces various types of bariatric surgery with positive influence on health status of second type diabetes patients. It also describes new trends in this field and takes into account activities before and after bariatric surgery such as preparation for the operation or post operational care. Practical part is dealing with the influence of bariatric surgery on body mass and metabolical parameters of the patients. For purpose of this study the research was conducted, representative sample of 162 patients was chosen, 68 people suffered from diabetes, the rest did not. Patients of both groups have reduced their weights significantly after the bariatric surgery of all kinds. The surgery with the biggest second type diabetes mellitus compensation is gastric bypass.
