Motivace uživatelů opioidů ke zneužívání Suboxonu

Abstract

Background: In the last few years the number of buprenorphine users has been increasing, as well as the number of Suboxone users in substitution treatment. Suboxone became the most used preparation based on buprenorphine in 2015. Thanks to the naloxone component any abuse in the form of intravenous application should not appear. But from the practical experience we know, that the naloxone component is not always working and we can observe its abuse in the form of intravenous application, as well as its black market distribution. This issue is discussed among experts rarely. Aims: The main aim of this study is to describe the motivation of opioids users to abuse Suboxone, either with intravenous application or its distribution to the black market. Next aim is to identify and describe the aversive effect that is connected to intravenous application of Suboxone, specifically how the aversive effect can evince and in what degree it is present. The aim is also to find a possible link between the intravenous application of Suboxone and the needle fixation phenomenon. Východiska: V posledních letech narůstá počet uživatelů buprenorfinu a narůstá i počet uživatelů Suboxonu v substituční léčbě, ten se v roce 2015 stal nejvíce užívaným přípravkem na bázi buprenorfinu. Díky naloxonové složce by nemělo docházet k jeho zneužívání formou intravenózní aplikace. Ze zkušeností z praxe ale víme, že naloxonová složka ne vždy funguje, ke zneužívání formou injekční aplikace dochází a stejně tak i k jeho distribuci na černý trh. Tato problematika není ale odbornou veřejností příliš diskutována. Cíle: Hlavním cílem práce je popsat motivaci uživatelů opioidů ke zneužívání Suboxonu, ať už formou intravenózní aplikace nebo distribucí na černý trh. Dále si práce klade za cíl zjistit a blíže popsat averzivní efekt spojený s intravenózní aplikací Suboxonu, konkrétně jak se averzivní efekt může projevit a v jaké míře se dostavuje. Dalším cílem práce je zjistit možnou souvislost mezi intravenózní aplikací Suboxonu a fenoménem závislosti na jehle. Metodologie: Ke sběru dat bylo využito strukturované interview na základě dotazníku. Výzkumný soubor tvoří klienti tří pražských certifikovaných substitučních center.
