miRNA, biomarker, axial spondyloarthritis, rheumatoid arthritisÚvod: MikroRNA (miRNA) jsou malé nekódující jednořetězcové RNA podílející se na inhibici genové exprese na posttranskripční úrovni, čímž ovlivňují řízení všech buněčných funkcí. Jejich dysregulace se podílí na patogenezi onemocnění, včetně revmatických. MiRNA se mohou vyskytovat i extracelulárně v tělních tekutinách, čímž představují nadějný diagnostický a prognostický ukazatel. Cílem práce bylo analyzovat expresi miRNA jako potenciálních biomarkerů stadia a aktivity nemoci a predikci terapeutické odpovědi u dvou nejčastějších zánětlivých revmatických onemocnění: spondyloartritidy (SpA) a revmatoidní artritidy (RA). Výsledky: Prokázali jsme odlišnou expresi cirkulujících miRNA u pacientů se SpA, které se lišily podle závažnosti axiálního postižení a/nebo korelovaly s aktivitou nemoci. Míra poklesu cirkulující miR-145 v plazmě po 3 měsících léčby TNF inhibitory u pacientů s ankylozující spondylitidou predikovala dobrou terapeutickou odpověď a dosažení nízké aktivity nemoci po roce léčby. Hladiny cirkulující i buněčné miR-125b v periferních mononukleárních buňkách (PBMC) byly u neléčených pacientů s časnou RA nižší v porovnání se zdravými kontrolami a stoupaly po zahájení terapie. Background: MicroRNAs (miRNAs) are small non-coding single-stranded RNAs involved in the posttranscriptional inhibition of gene expression and thereby regulating all cellular functions. Their dysregulation contributes to the pathophysiology of many diseases, including rheumatic diseases. MiRNAs can also be found extracellularly in body fluids and represent promising diagnostic and prognostic biomarkers. Our study aimed to investigate miRNAs as biomarkers of stage and activity and predictors of therapeutic response of two most common inflammatory rheumatic diseases: spondyloarthritis (SpA) and rheumatic arthritis (RA). Results: We found several circulating miRNAs differentially expressed in SpA patients reflecting the severity of axial involvement and/or disease activity. The decrease in circulating miR-145 in plasma of patients with ankylosing spondylitis 3 months of anti-TNF therapy predicted a good therapeutic response and low disease activity after a year of therapy. Circulating and intracellular expression of miR-125b in peripheral blood mononuclear cells (PBMC) was lower in treatment-naïve patients with early RA than in healthy controls.
