Péče o pacienta po transplantaci jater

Abstract

Svoji diplomovou práci jsem psala na téma Péče o pacienta po transplantaci jater. Práce je rozdělena na praktickou a teoretickou část. V teoretické části jsem se zaměřila na anatomii a funkci jater a žlučových cest, na jaterní onemocnění, která byla indikací k transplantaci jater pacientek, jejichž kasuistiky vypracovávám v praktické části. Uvádím jejich etiologii, klinický obraz, diagnostiku, léčbu a komplikace. Zvláštní kapitola je věnována problematice transplantace jater. V praktické části jsem zpracovala kasuistiky dvou pacientek po transplantaci jater hospitalizovaných na Klinice anesteziologie, resuscitace a intenzivní péče v IKEM. V první kasuistice popisuji průběh hospitalizace u pacientky s diagnózou jaterní cirhózy. Ve druhé kasuistice popisuji průběh hospitalizace u pacientky s akutním selháním jater na podkladě Wilsonovy nemoci. Nejprve jsem vypracovala průběh jednotlivých z hlediska léčby významných dnů, odebrala jsem ošetřovatelskou anamnézu, na jejímž základě jsem stanovila potřeby pacientek a poté jsem popsala ošetřovatelské intervence. V přehledu jsem uvedla ošetřovatelské diagnózy. Ošetřovatelskou péči jsem zhodnotila a celý průběh hospitalizace shrnula. I wrote my thesis about The care of patient with transplant liver. The work is divided into theoretical and practical part. I directed the theoretical part to anatomy and function of liver and gall-bladder ways, liver's diseases, which conducted to transplant of liver patiens, whose case reports I will work in the practical part. I present etiology, clinical symptoms, diagnostik, complication and treatment of diseases. I paid attention a special chapter to problematic of transplant of liver. In the practical part I work out case report sof two patiens with transplant of liver, who were hospitalized in Clinical anesteziology, resuscitation and intesive care in IKEM. In the first case report I describe course of hospitalization of patient with liver cirrhosis. In the second case report I desribe course of hospitalization of patient with acute hepatic failure on the strength of Wilson's disease. At first I work out a course of important days for treatment.I took nursing anamnesis. I set needs of patiens and then I describe nursing interventions. In summary I present nursing diagnosis.I evaluated nursing care and whole course of hospitalization. At the end I compare a course of treatment about the patiens with different diagnosis.
