postoperative epidural fibrosis, neuromodulation, chronic painÚvod Epidurální fibróza (EF) je definována jako nefyziologická tvorba jizev, obvykle v místě neurochirurgického přístupu do páteřního kanálu, v těsném sousedství a kolem vzniku spinálního kořene. EF se od počátku chová jako reparační zánět, způsobuje zpravidla příznaky charakteristické povahy a klinického průběhu (neuropatická bolest). Léčba epidurální fibrózy, vedoucí k failed back surgery syndromu (FBSS), neuromodulační technikou je velmi drahá. Nalezení vhodných parametrů pro indikaci léčby je proto velmi důležité. Cíl Studie je založena na důkazu významu epidurální fibrózy pro rozvoj chronické neuropatické bolesti. Výzkum je zároveň zaměřen na srovnání rozsahu fibrózy a účinku stimulace (míšní stimulace-SCS). Cílem studie je najít vhodný výběrový faktor pro indikaci neuromodulace. Metodika I. dvojitě slepá prospektivní studie byla provedena s cílem prozkoumat soubor 200 pacientů vyžadujících chirurgickou léčbu výhřezu meziobratlové ploténky (hernie disci intervertebralis ). Pacienti byli náhodně a slepě rozděleni do 2 skupin: první skupina s peroperačně lokálně aplikovanou směsí obsahující kortikosteroidy, druhá skupina bez aplikace směsi. Všechny parametry splňovaly podmínky dvojitě zaslepené studie, včetně statistického vyhodnocení výsledků, v průběhu diagnostických a terapeutických postupů....
Background Epidural fibrosis (EF) is defined as nonphysiological scar formation, usually at the site of neurosurgical access into the spinal canal, in intimate vicinity to and around the origin of the radicular sheath. From the very onset, EF behaves as a reparative inflammation causing, as a rule, symptoms of characteristic nature and clinical course (pain). Treatment of epidural fibrosis causing failed back surgery syndrome (FBSS) by neuromadulation technique is very expensive. Finding of suitable parameters for the indication of treatment is therefore very important. Aims The study is based on evidence of the importance of epidural fibrosis for the development of chronic pain. Research is also focused on the comparison of the range fibrosis and the effect of stimulation (spinal cord stimulation - SCS). The goal is to find a suitable selection factor for the indication of neuromodulation. Methods I. A double-blind prospective study was conducted to investigate a cohort of 200 patients requiring surgical treatment for intervertebral disc hernia (hernia disci intervertebralis).
